Where to start

Already receiving care?

Living in an aged care home

If you are living in an aged care home and you are on transition care leave, the subsidies to your residential aged care provider (more information) continue to be paid by the Government. This ensures your place is held for you until you return from transition care or until an appropriate alternative care place is found.

Receiving community care

If you are already receiving a Community Aged Care Package (CACP) (more information), Extended Aged Care at Home (EACH) package (more information) or Extended Aged Care at Home Dementia (EACH D) package (more information), the subsidies to the community care provider continue. This ensures that your place is held until you return from transition care.
