Question: How do I know if I 'm eligible for any of these programs?
Answer: Most home care services have some sort of assessment to see what care and support would best meet your care needs. The Commonwealth Carelink Centres can provide advice on what care services area available in your area. An ACAT assessment is for people who have more complex needs and need a more comprehensive assessment of their situation. An ACAT assessment is not required for services such as those under the Home and Community Care (HACC) program, but is required to determine if you are eligible for: • An aged care home (to go in permanently or for respite care); • A Community Aged Care package (CACP); • Extended Aged Care at Home (EACH) and EACH Dementia packages; and • For transition care services.
