Question:	I have complex chronic health problems. Most of the time I am reasonably well and manage at home. How can I get looked after at home at the times I am unwell?
Answer:	You can call 1800 200 422 in the first instance to identify the services that might best suit your needs and are available in your area (i.e from needing a little bit of help through the Home and Community Care Program, through to needing more complex and or coordinated care through Community Aged Care Packages).
